Cortisone And Other Glucosteroids Increase The Risk Of Having A Pulmonary Embolism.

This week, doctors have shared another risk factor for pulmonary embolisms, DVTs, and other types of venous thrombosis.  The good news is that we now have another risk factor that we can avoid.  The bad news is that the risk factor is a critical treatment for many patients.  Glucosteroids, one of which is cortisone, have been found to increase the risk of having a venous blood clot by two to three times.  Even more problematic is that the form of the steroid doesn't matter.  Inhaled, injected, or swallowed, all glucosteroids increase the risk of a clot (although some forms are more risky than other forms).

Doctors are already cautious about using glucosteroids due to their lengthly list of frequent, serious side effects.  This caution should continue.  The risk of clots is the highest after beginning a new steroid regimen.  The risk continues throughout the steroid use and then for at least a year after the steroid is discontinued.

The important thing to learn from this study is that every patient needs to discuss this risk with his or her doctor.  It would be a good idea to discuss the risk of the steroid, other risks that the patient has, and possible ways to lower the risk of clotting.

I had cortisone injections in both feet for plantar fasciitis 24-48 hours before developing a blood clot in my right leg.

While no doctor has told me that cortisone injections are a significant risk factor, I have been suspicious that nerve blocks caused mine. I have been on cortisone several times since then but it makes me wonder.

I was diagnosed with my first blood clot 8 days after a cortisone injection for plantar fasciitis. Every doctor I have spoken with says they are unaware of this risk factor, but given that it was an ultrasound injection which went right over the area which developed the clot, (meaning it wasn't there prior to the injection) I think the shot itself or the more-sedentary-than-usual recovery period led to the clot. I had all the blood clotting/factor tests done after the fact and was negative for all. We need to get the word out about this so docs and patients know. Scary stuff!

I came across this site looking for answers as to why at such a young age I had a heart attack caused by a blood clot just 3 days after 4 facet joint injections, 1 nerve root sleeve injection and 1 epidural injection. If anyone has found any further research into blood clots after these sorts of injections i would love to hear about them.

Have you been evaluated by YOUR own chosen physician/speciaslist? You are alliwed to see one or more or more private docs, and if your doc(s) disagree w/the W.C. doc(s) then only you can fight for yourself. If there are ride is you want to avoid, such as blood clots, then it's you, your docs agasinst W.C.!! W.C. docs are employed by W.C. to advocate for W.C. to cut doiwn on W.C. payouts...the sooner they can yet you back on your feet, with side effects and risks included. Then they get you back to work sooner. If there's a flare up or new injury, they will try to make it like you're to blame.

I have been on cortisone injections since 2010 I got a bilateral pulmonary embolism in 2015. I had no idea what may have caused the clots as all my tests came out fine. And I just got injections last week so this means I have to wait a year to get off the blood thinners?

I had spinal facet burns and spinal steroid epidural done. A week later I had some chest pain and slight breathing problems. A few days later it felt like I tore a chest muscle. Next day I was in the ER with PE. And I was screaming it hurt so bad. I’m having all kinds of tests done. I don’t know what caused it but I’m off the pain management and on blood thinners....

I had bunions off on both feet, I now have blood clots UNDER the left foot, doctor at the hospital asked if they were giving me problems?? At the time no, but now I have tingling sensations and can't bend my big toe on either feet, I feel this lump under my foot. Doctor say they either use cortisone injections or surgery to get rid of them!! Lovely, after reading all of these replies I wonder how these doctors get their degrees.!! no in injections for me thank you!!

I had cortisone injections in both knees and within several days experienced severe ankle pain. I was dismissed by my doctor and sent home. The next day I had excruciating calf pain. A clot. I began treatment and several days later the clot broke and one went to each lung. It was caught and I was lucky.

I would like to also add my Hematologist told me anytime you get any kind of an injection there is a small risk of clots but I am 99.9% sure the Cortisone caused all of ours or maybe the Cortisone and the injection together. I also would like to let you all know I have spoken to quite a few people on Facebook in the same situation as us. They all had unprovoked clots but were getting Cortisone injections at the time. I don't know if doctors want to keep it quiet or if they simply don't know but I am sick and tired of being told that Cortisone doesn't do it. in a way its my fault I got several injections sometimes 8 at one time but I was in horrible pain but I will never get my life back now because I didn't research Cortisone and its effects. Do we take a chance and get off the blood thinners and hope we don't clot again? Do we stay on the blood thinners forever? These are the questions that I have and no one can help me answer. Its time we crack down on the makers of Cortisone and in my opinion we should sue them.

I had a cortisone injection into my right foot for sesmoiditis, this was the second of its kind within 6 months. A couple of weeks later, I had a dvt in my right calf & developed a submassive bilateral pulmonary embolism. I survived only because my husband came home early & found me collapsed.

Sorry to hear that I had been getting the cortisone injections for some time years actually and then all of a sudden in 2015 it hit me and I got a Bilateral PE. Reading all of your comments this cannot be a coincidence we need to get to the bottom of this so we can know if we are on blood thinners for no reason. I stopped the injections almost a year ago and I am done I want it out of my system for good.

I just can’t understand why medics are not following and/or studying this occurrence, it’s just too much of a coincidence, to be a “ one off”. I had a PE back in 1998, just as a curiousity, I asked my Gp to see if I’d had any cortisone etc, I’d had cortisone injections in both my shoulders, plus a course of predisalone in July of that year. I’m sorry this is too much of a coincidence for me!

This is far more than just coincidental, this really should have been picked up earlier. Since my bilateral PE’s last June, I went for my 6 month check up ( 1st check, as they forgot to give me a 3 month app!!!), Dr wasn’t happy with how I was feeling, so sent me for an echo, during that, the cardiologist who was looking over my scan, as the radiographer, was doing it, stopped. Both started going back over a certain area, I knew there was something wrong. They found a “Mass” on the back wall of my left atrium, and I was sent up to ICU, apparently I was, and am, in heart failure. I have been devastated, by this diagnosis, as you can imagine. My preferred outcome, I think, is that it will be a clot. They wouldn’t believe that I’d not missed one dose of my Riveroxiban, I’m now on warfarin. All the tests have now been completed and I’m awaiting a diagnosis and how or what, is the way forward.
It has also been revealed, that the clots I had 20 years ago, were due to Predisalone & cortisone injections, into my shoulders, just weeks before.
What we need our medics to do, is to be aware of these highly dangerous, if not fatal consequences-I know & respect that they are not machines, and can’t be expected to remember everything-but, we have excellent software that can remember everything, and it is this software, that should be shouting out loud, for any contraindications or concerns, with each individual patient. Seems simple I know, the software is only as good as the people who input data, but surly-just one click!!!

I too have experienced a dvt and a massive PE. I was in great health but I have arthritic knees. Once a year on the Wednesday before Thanksgiving I get a routine cortisone injection. Three weeks later I find myself in surgery and doctors in shock that I wasnt dead from the large clot blocking all the ventricles of my heart. Clots in my lungs and a huge one in my leg. I was told that aI may have an underlying cancer that hasn't been diagnosed. I webt to the oncologist and he started asking me questions and he asked what was going on weeks prior to the hospitalization? Did I have any injuries? I just mentioned the injections and he immediately knew the culprit. Still on the blood thinners and unsure for how long.

After reading all these replies I'm in the same boat. I had the series of three lower back cortisone injections. Also had facet injections on cervical and lower back. A former pro athlete have been healthy my whole life. Now I have also had two pulmonary embolisms and DVT after getting off blood thinners six months later feeling happy, I found out I had full blockage artery in my heart and had to go in for a stent after pushing through silent heart attack. I don't know anything else that could have caused this in my body I lead such a healthy lifestyle! My body was so strong I perform my own bypass and grew artery around blockage my cardiologist doctor was amazed. also notify doctor after every injection 6 hours later my legs would go numb or collapse every time!

This is shocking news. Thanks to all of you. I had 3 steroid injections in the lower spine, due to herniated discs in L4/L5 and L5/S1, over a 7 week period Sept / Oct 2019. The nerve pain radiated through my left hip and thigh, to the knee and additional tingling of nerves in my calf/foot Two weeks after the 3rd injection I noticed deeper indentations on my left ankle from my socks at the end of the day as compared to very little on my right ankle.
I was curious but not alarmed.
The swelling of my ankle and foot was not pronounced at that time.
However, during a routine exam at my GP two weeks later, on Nov 21, he noticed the now-increased swelling of ankle / foot / toes and wanted an ultrasound done on my left leg as soon as possible. I had the ultrasound the next morning and the tech alerted me to one blood clot under the knee and at least two more in my calf area. She asked me several times if I had any breathing problems or chest pain. I said No.
I was stunned by the news as I have been exercising and in good health all my life, now in my mid-60's. The ultrasound clinic immediately contacted my GP and by the end of the day I was starting Eliquis 5 mg, 2 per day, to "treat certain types of blood clots DVT/deep vein thrombosis and PE/pulmonary embolus and to prevent them from forming again. It works as an anti-coagulant that works by blocking certain proteins in your blood."
I do not see my other doc who did my injections until early Dec.
Wow, do I have I news to tell him. I hope he's not clueless.
I had no idea that the clots in my leg were a result of my lower back steroid injections until I checked the internet for a possible connection yesterday and found this website. I am so thankful it was available and to all of you for sharing your helpful stories.
I read sites prior to my injections to check for adverse reactions after injection and none listed blood clots as a possible problem.
We are certainly not living in the early 1900's with limited connection to each other's information and concerns. It appears the medical community is not listening to our voices. The first reply on this site was in 2015, and the last was less than 10 days ago, Nov 2019, so there have been plenty of us raising awareness and concerns about this life threatening side effect from steroid injections. Are there any medical journal / dissertation studies published on this subject that are available on the internet ?

I am writing again after 3 months after my first comments on 11/24/19.
In early Dec. 2019, I had an appointment with the doctor who gave me the 3 steroid injections and I told him about the clots in my left leg. As expected, he knew of no connection between the injections and the clots.
I have continued to search for other ways I may have developed clots, other than the standard medical answer of "you became more sedentary". Well, of course I did, with herniated discs causing serious pain from my lower back down to my lower leg, I was not walking as much as before ! On the other hand, I was never walking less than a 1/4 mile on my most "sedentary" days.
Today I have been reading more on NSAIDs versus Tylenol type pain relief. I continued to be stuck with only Tylenol while on Eliquis to control these clots. I wondered about the old-fashioned aspirin (an NSAID) thatI had been on for years, which was162 mg per day as a blood thinner to prevent heart attacks ( my dad had 3, the last one killed him at age 45 ).
So being curious, I Googled "Aspirin" and found an article by the Mayo Clinic, dated Jan. 9, 2019, at
www.mayoclinic.org>in-depth>daily-aspirin-therapy>art20046797.
Those of us who have been on aspirin regimens and directed to stop our daily doses in order to receive injections or procedures or surgeries must read this short article, especially the part that I include below:
" If you have been taking daily aspirin therapy and want to stop, it's important to talk to your doctor before making any changes. Suddenly stopping daily aspirin therapy could have a rebound effect that may trigger a blood clot. " I did not know this until today.
As directed by the doctor, I completely stopped my daily aspirin therapy about 2-3 weeks before he gave me my first steroid injection Sept. 2019. Last Injection October 2019. Once the clots were discovered in Nov. 2019, I started Eliquis and told to stay on Tylenol .... and not take Aspirin / other NSAIDs.
I return for an ultrasound in 5 days to see if the clots are gone.
This is just another segment in this discussion. I will keep reading.
Thanks again to everyone here for helping me understand more.

I had an injection in my knee (a workman comp issue). Shortly after, I developed ankle and then a pain in my calf (opposite leg). Then I almost passed out at work. I went to the doctor and they found a huge clot in my leg and several large clots in my lungs. Now I have to be on blood thinners that cost $500+ per month. Why didn't they discuss the risks with me before administering the shot. The doctor is denying that the injection is the source of my problems. I know that it was from that. I have had no prior issues with my health or clotting. This one shot may have ruined my life, but at least I am still here to tell my story. Workman comp is not willing to take responsibility for this matter even though this will be a health and financial drain now for the rest of my life. We need to speak out about this.

I had a bilateral nerve block in Jan 2020 near my anterior scalene muscle. Something didn't feel right so I went back to the Clinic, they did an ultrasound and found a clot adjacent to where the nerve block had occurred on one side only. This clott was not there 3 hours prior.

Kind of and actually REAL pissed right now. I had no clue!

My family doctor thought I had a rotator cuff tear. The orthopedic surgeon took an x-ray, saw no tear and has started to treat for bursitis.

On February 3rd 2020 the orthopedic surgeon gave me cortisone in my left shoulder. On February 6th 2020 I had a heart attack, followed by another one on February 8th 2020. I am 47 years old.

I KNEW my shoulder had something to do with it the whole time! I told this to my wife and to one of the ICU nurses. They KNEW I had the cortisone procedure the same week but NOTHING was said to the shoulder procedure relating to anything, let alone the heart attack.

When I got home from the Hospital, I just happened to see a tv commercial for some drug that helps with or helps avoid clots. Towards the end of the commercial they said something like, “do not take if you are receiving spinal injections”. I instantly related my shoulder injection to the heart attack, began researching and here I am.

When my heart attack ended, all of the pain (chest, neck, jaw) lessened to almost nothing but then gathered all in one spot. All of the pain I was having, gathered and continued (severely) just below where I had the injection a few days before. And it’s like the pain was a mass in my arm (something physical), not just pain. This is one of the reasons I still did not go to the hospital until the next day. A heart attack crossed my mind but I thought pain from the injection was radiating out to the rest of my body.

I have now read that the cortisone can crystallize in people. Maybe that is what I felt in my arm, after the heart attack, where all of the pain ended and remained? Is it the crystallization that causes the clots?

I asked my regular doctor (upon informing him of my heart attack) if the cortisone procedure could have caused or at least helped cause the heart attack. He said, he had never heard anything relating these injections to heart attacks.

I have my first follow-up this coming Monday the 24th with a Cardiologist. I will be taking this article and more importantly, all of our COMMENTS with me! I will NOT allow him to just brush it off. And honestly, how could he after reading the other comments.

Right?

Wow ! I was just diagnosed with (2) blood clots in my right calf and (1) in my right forearm. "Coincidentally," I just had a cortisone injection 10 days ago in my lower back (for spinal stenosis). I also had previously had the same injection in March, and one previously in 2017; also cortisone injections - 2 times - for trigger finger during the past year. Starting on a starter pack of Xeralto today. Now, I'm wondering/concerned about other areas of my body that may have clots - undiagnosed. I'm very active - 3 marathons and 2 triathalons - and absolutely not sedentary. So, strange...I had a 'fainting' spell in March 2 weeks after my spinal injection. At that time, doc said I had an issue with 10% - 20% blockage of plaque and put me on Plavix. Now, the whole picture is coming into focus, and I would not have put this into perspective, had I not come across this site ! Thanks for the new awareness.

I had cortisone injections in my heels on July 1st, then the following weekend I started having pain in my calf. On July 8th I was diagnosed with DVT in the same calf. It's interesting to see how many people have developed blood clots soon after cortisone injections. I haven't found a lot of published scientific studies on this but I keep looking.

I have had several cortisone injections in my lower back (L4-5,S1, Sacroliac Facet Joints, along with ablation of nerve branches of cluneal and facet regions. I was lucky to be in hospital for another reason 21/7-28/8. I had another cortisone injection early August, a week later I was diagnosed with 3 DVTs in left leg (side of injection) and 1 in right leg, with bilateral PEs (many, up to 2cm) in and past the 1st pulmonary branches. It was lucky I was in hospital as I had no symptoms except for shortness of breath about 4 days later. IT was investigated thoroughly for 2 days and when final diagnosis was made I was sent to ICU for 4 nights. Due to bloods showing no reason for the “unprovoked” clots I insisted upon PET Scan due this Thursday to investigate for cancer. I will be bring this thread to the attention of my specialist.
Feeling a little less crazy - all I’ve wanted is to know “why”.

My husband had an injection in his knee (hyaluronic acid/gel) 3 months ago. He developed swelling, redness and flaking of his leg 3 days later. He had developed a massive blood clot. Three months later and 2 doctors, 2 specialists and a surgeon later, there are still no real answers. The clot is from his knee to his ankle. He has been on a blood thinner and will continue to be. Today the surgeon told him she couldn't believe no one has treated his skin condition that developed on his leg as a result of the clot. She is saying the color of his leg will never really be back to normal. Once the swelling goes down he will start the compression socks. I can't believe all that I am reading here. Thanks for sharing your stories.

I was given cortisone shots in both heels and literally could feel it moving upward through my body because I went numb all the way to my head. I began to feel exhausted and out of breath alot and was chalking it up to the heat. I was having to take four to seven hour naps each day for about a week. My calve began aching and felt like I was on the verge of a charley horse constantly. THANK GOD my GP listened to my concern and sent me for a leg scan. It showed clots in most of the veins in my right leg. She then had the foresight to send me for a lung scan which showed a massive saddletype pulmonary embolism which had also extended to all three lobes of my right lung. I was hospitalized and they inserted a vena cava filter into my chest the next day to try and prevent any more clots from travelling to my heart, lungs or head. I feel lucky to be alive but extremely angry because all of this could have been prevented. When I mentioned the new diagnosis and what I had been through to my podiatrist and that there was evidence that cortisone shots cause bloodclots, I could tell he knew this by the look on his face. He cut our visit short, hurried out of the room and had the nurse finish things up. I now wonder how many have died needlessly or who are damaged for life physically and monetarily due to this???

This is just incredible that many doctors recommend cortisone injections without warning patients about this possible side effect (maybe most don’t know?). I had a cortisone injection in the shoulder for “frozen shoulder”, after one week I had a superifical thrombophlebitis on the small saphenous vein, I was very active in sports, healthy and 35 yo. Now I have been damaged phisically and menthally, none will change this and I’ll have ti deal with it (I have become paranoid everytime my calf hurts, I have pain after some time that I walk or bike, so life has definitely changed in bad). I wonder, why this information that cortisone shots can lead to DVT or SVT are not spread and delivered to any old and new orthopedic doctors?

3 cortisone shots in left foot in late Fall 2020. On Christmas Eve, I was in ER with clots in my lower left leg. Next day, clots found in lungs. Now I am on Eliqus and hoping for the best.

I had a cortisone injection in both knees due to pain from years of wear and a recent meniscus tear in my right knee and 10 days later I awoke to pain in both legs. I had redness, knots and hot spots on my left leg above my knee on the inside of my thigh and below the knee near my calf(3 spots in total). My right knee hurt right below it towards the inside of my leg and was another hotspot, knot. I went to the ER and was found to have a DVT behind my right knee and superficial blood clots everywhere else. I was put on a blood thinner (Xeralto) which cost me over 600.00 a month. All of this happened after I lost my job due to COVID and I was without insurance. I am on Warfrin now due to costs of Xeralto and just having the biweekly INR's is a pain in the butt. When I called my Orthopedists to cancel my 3 month injections, the front desk person asked me to have my records from the ER sent to them because the doctor would want to know. Had I not found this thread the night before I went to the ER, I may not have gone and just figured it was a reaction to the shots. I have had a couple cortisone shots in my elbow about 20 years prior and had no problems.

The answer is, of course, our favourite cup of coffee. Coffee can be found in a wide range of foods and beverages, such as cold drinks, cakes, and biscuits. However, the concentration of coffee or caffeine in them is insufficient to have any effect; they are only for flavour.

My husband had been given 2 shots (steiroid) for his eczema flare ups 2 weeks from each. Now he's been in bed for 5 straight days with a blood clot on his leg. Also pinkish colored urine so i'm sure there are alot more underlying problems that will start to show themselves. My heart goes out to all of you. Terrible to go through and also to watch someone go through it. What is the answer to it all?

I received a cortisone injection in my right hip 11/8 and another one in my lumbar region 11/18. On 12/21 I suffered a bilateral pulmonary embolism.

I was diagnosed with mild heart conditions in November/December and prescribed Furosemide and Potassium, later Metoprolol. I also was checked 3x for blood clots (by ultrasound twice and d dimer was run once) as I had ankle injuries not properly cared for, for awhile and my foot got very painful, swelled and froze up. I had my first corticosteroid injection in my subtalar joint (below the ankle) on January 19th. I began to suffer from major muscle cramps in my feet and legs. I had an extremely painful cramp in my left thigh early on 1/22. On 1/30, I began to have sharp chest pains. I had a nuclear stress test on 1/31 and 2/1 which showed an abnormality in my left lower part of my heart and I was scheduled for a heart catheterization on 2/11. Near collapse with the sharp chest pains and from ongoing imbalance (different medical issue), I was brought down to the ER, admitted when symptoms couldn't be resolved and had my heart cath the next day. Results were good but symptoms continued. My PCP in hospital follow-up, at the last minute realized the hospital didn't run a d dimer test so she ordered it. It was 3860. She called me late that night to go to the ER and three hours later, a PE was found by CT Scan in my lower lobar area of my right lung. Doctors focused on connective tissue disease as the cause (history of positive bloodwork for SLE). Rheumatologist ran 3 blood tests, all negative. Still no reason for clotting but my ankle is telling me it is time to consider another corticosteroid injection. (I was told I can't take the go-to pain med, Naproxen Sodium, while on anticoagulants.) I am at a loss of what to do, but will need to consult with my foot/ankle specialist soon.

My Orthopedic Surgeon gave me a cortisone shot in my shoulder to treat severe pain due to torn tendons and advanced arthritis. I am very cautious about taking any drug and when I asked about the risk factors I was told it was a mild, low dose form and the risk was very minimal. The shot did not help and a month later he ordered a second shot of a more powerful cortisone formula to be performed by a specialist. After reading about the risk factors online I refused. As my pain issue increased my Ortho said the shot was the only way for him to proceed to evaluate my pain and determine when and how reparative surgery could be done. I questioned him and the injection specialist about the risk factors and voiced my concern. I was told that since I had not had numerous injections that the risk factors were very minimal. I was NEVER told of the risk of DVT'S. Following the injection I had severe shoulder pain, nausea, dizziness and insomnia for a week. On the 6th day I woke up to pain in my right calf. I was diagnosed with a DVT and put on blood thinners. When I asked my Dr's if the cortisone could be a contributor to my dvt I was told no. Prior to the injection my blood work and overall health had always been excellent. I had been told repeatedly by dr's that my health was excellent and I was at very low risk for any clotting issues (stroke, heart, ect). Since I was diligent to ask about the risk factors I now know my Dr's flat out lied to me. If I had seen the above posts prior to my injection I would have refused it. If I can easily find this information online then I find it very hard to believe that Dr's are not aware of the risk and are being reckless and dishonest towards their patients - most likely for profit. I would rather live with shoulder pain than to now have to worry about a potentially life threatening clotting issue. It will be a very long 3-6 months before I know this poison is out of my system. If you are reading this DO NOT TAKE CORTISONE. I sympathise with everyone on here who has been victimized by our corrupt medical system.
